Job Description:
This position provides clerical and administrative support to enhance integrated care management for targeted populations and individuals. Facilitates communication with payers and authorization of services in accordance with regulatory and contractual requirements, patient and family-centered interdisciplinary care planning, effective care coordination, collaboration and communication between healthcare settings, and effective transitions to the appropriate level of care.
The Care Manager Assistant is responsible to facilitate all (Inpatient and Outpatient) initial Home Health Care referrals. Assist the inpatient providers efficiently as a resource in order to achieve the goals of the inpatient program. Assist hospital based team with utilization and discharge planning functions for inpatients in the acute care setting. Facilitate the continuum of care referrals and transfers to the most appropriate level of care that is least restrictive and most cost effective that provides a safe environment.
Essential Functions

- Contributes to and supports the corporation’s quality initiatives by planning, communicating, and encouraging team and individual contributions toward the corporation’s quality improvement efforts
- Responsible to verify eligibility and demographic information of hospitalized/post-acute patients.
- Responsible to gather information related to hospitalized/post-acute patients in relation to continued stay review, simple discharge planning and utilization review.
- Responsible to assist the RN Care Managers with moving the patient through the continuum by communicating with and completing tasks as directed by the RN or supervisor.
- Responsible to assist the case management staff with implementation and documentation of the discharge care plan that includes initiation of referral forms, data gathering clinical information, chart copying, direct patient contact, PCP and specialist appointment scheduling.
- Responsible to anticipate discharge planning needs and initiates appropriate referral forms.
- Responsible to coordinate faxing of referrals, orders and face sheets to applicable offices or referral sources.
- Responsible to assists inpatient team with opening charts and arranging home health.
- Responsible to act as a liaison between physician and RN Care Managers as needed.
- Responsible to adhere to HCP, customer and supplier confidentiality and HIPAA guidelines at all times.
Experience Requirements
Minimum: 2-5 years clinical experience; LPN, MA, CNA, Medical Receptionist, or equivalent
- Medical Assistant preferred**
Educational Requirements
High School Diploma or GED
Knowledge, Skills, Abilities
Knowledge of medical terminology
Strong interpersonal and communication skills
Organization & problem-solving skills
Computer literate with multiple systems
Ability to function in a diverse, rapidly changing environment and prioritize

Minimum Qualifications
Two years of healthcare experience.

Advanced computer skills (i.e. developing spreadsheets, creating charts and graphs, word processing, process flows, complex formatting, data manipulation, creating and running reports, creating presentations, and using multimedia content).

Two years of experience working in an office setting, working in a clerical position with keyboarding and data entry responsibilities, or working in customer service.

Experience coordinating projects.
– and –
Demonstrated interpersonal verbal and written communication skills including being proficient in spelling, punctuation, grammar, and other English language skills.

Demonstrated basic math skills.
Physical Requirements:
Interact with others requiring employee to verbally communicate as well as hear and understand spoken information.

Operate computers, telephones, office equipment, including manipulating paper requiring the ability to move fingers and hands.
